Lithium orotate is the lithium salt of orotic acid (vitamin B₁₃). In this compound, lithium (Li^+) is bound to orotate (the deprotonated form of orotic acid) rather than to carbonate as in prescription lithium carbonate. Lithium orotate is available as a nutraceutical supplement, marketed for mood support and neuroprotection at low doses. Pharmacologically, lithium orotate delivers lithium ions that, once dissociated, have similar biochemical effects to pharmaceutical lithium: they can modulate neurotransmitter signaling, enhance neurotrophic factors, and inhibit glycogen synthase kinase-3 (GSK-3). Some proponents suggest lithium orotate may be more bioavailable or better at crossing the blood-brain barrier, although this is not conclusively proven. Regulatory note: Unlike lithium carbonate, lithium orotate is not an FDA-approved drug; its use is based on extrapolation of lithium’s known effects. Research from the 1970s–80s (limited in scope) indicated possible benefits in alcoholism and mood stabilization. However, rigorous clinical data are lacking, and concerns exist about kidney accumulation of lithium with chronic use (as with any lithium source). Each 131 mg of lithium orotate contains about 5 mg of elemental lithium. Users should exercise caution due to lithium’s narrow therapeutic index.
